This is a two step process, as follows:

*If there are multiple individuals on your "side" of the transaction, only one person is needed to complete the questionnaire.

 

Step two: electronically sign the disclosures/authorizations. 

*Everyone needs to perform this task. You will receive an email with instructions for e-signing.
